Improving safety

It is not just the Indian Navy alone to have suffered submarine safety. The US Navy too underwent similar problems till 1963 when it introduced the SUBSAFE system, a quality assurance programme applicable to planning, design, manufacture, installation, operation and maintenance. After the Columbia disaster, NASA was in favour of SUBSAFE.
In fact, it is necessary to introduce such methods in all engineering departments in India to improve safety.
