Pre-operative Transcatheter Arterial Embolization, Resection of Large Adrenal Tumor

JAMMU: All India Institute of Medical Sciences (AIIMS) Jammu has achieved a significant milestone in advanced cancer care with the successful management of a highly complex surgical case involving a large left adrenal tumor.
The procedure, carried out by the Department of Surgical Oncology, demonstrates the institute’s growing capability in handling challenging oncological conditions through a coordinated multidisciplinary approach.
The patient had initially been diagnosed outside the institute with non-specific symptoms and imaging findings suggesting a left adrenal tumor. After consulting multiple medical centres, the patient eventually arrived at AIIMS Jammu for further evaluation and treatment. At the institute, a series of comprehensive investigations including advanced imaging and biochemical tests were conducted. These confirmed that the tumor was functional in nature, meaning it could produce hormones that may lead to sudden spikes in blood pressure during surgery, posing serious risks to the patient’s life. Considering the complexity of the case, a multidisciplinary tumor board meeting was convened to carefully plan the treatment strategy. The Department of Endocrinology played a crucial role in preparing the patient for surgery. Under the guidance of Juhi Jamwal, Assistant Professor, the patient underwent pre-operative optimization with alpha and beta blockade therapy. This stepwise medical management helped stabilize the patient’s blood pressure and heart rate, which is essential before undertaking surgery for functional adrenal tumors.
The patient was also suffering from Chronic Obstructive Pulmonary Disease, which required careful respiratory management. Ganesh Kumar, Assistant Professor in the Department of Pulmonary Medicine, ensured that the patient’s lung condition was adequately optimized to allow safe anesthesia and surgical intervention. In a rare and highly specialized procedure at the institute, Shariq from the Department of Radio Diagnosis performed pre-operative trans-arterial tumor embolization. This advanced technique involved blocking the blood supply to the adrenal tumor, significantly reducing the risk of heavy bleeding during surgery and improving the chances of a successful outcome. Following this crucial step, the patient underwent a left adrenalectomy with splenectomy performed by the Surgical Oncology team led by Paras Khanna, Assistant Professor. The procedure was conducted with the support of the anaesthesia team including Shruti Sharma, Additional Professor, along with Assistant Professors Slomi Gupta and Sandeepika Dogra. Their expertise ensured patient safety throughout the operation.
The ICU nursing staff and ward teams also played a vital role by providing continuous monitoring and dedicated care during the recovery period. After a week of post-operative observation, the patient showed steady improvement and is now clinically stable and ready for discharge.
Congratulating the medical teams, D. N. Sharma highlighted the importance of multidisciplinary collaboration in managing complex oncological conditions.
